Your feedback is incredibly important to us. We want to give patients the opportunity to report any incidents when they are dissatisfied with their care at MarillacHealth. Patients can submit a confidential incident/concern report by clicking on the link below. Your concern will be reviewed by our Quality and Compliance team.
Please know by submitting a report, it will not result in any adverse action or negative treatment. If you have any concerns that cannot be resolved with your care team, please contact us here. This incident reporting is not intended to resolve medication refills or questions, those questions are to be directed to your provider and the care team.
